What we offer
Service Coordination
Your single point of contact to organize providers, schedule visits, and keep your child and family’s goals at the center.
Family Training (Special Instruction)
Coaching for caregivers during everyday routines • Mealtimes, play, bath, etc. Allowing skills to build where life happens.
Therapy Connections
We get your child connected with speech, OT, PT, and more through BabyNet/OIDD (formerly DDSN) pathways.
Assistive Technology
Support exploring tools that help little ones participate, communicate, and thrive.
How it works
Referral
Families and professionals can refer through BabyNet (birth–3). If your child is transitioning to OIDD (formerly DDSN) (up to 6), we facilitate this transition every step of the way to ensure that your child continues to receive all services needed, and continue supporting your family through age 6. .
Intake & Eligibility
We’ll gather your story, coordinate evaluations, and confirm eligibility through the state systems.
Family-Centered Plan
Together we build goals that fit your child and your routines.
Ongoing Coaching
Visits happen in natural environments such as your home, daycare, and community with caregivers as the constant team members.
